Visitation Form Rejection - so nervous

I had a small first time misdemeanor drug offense 4 years ago....will this affect my chances of seeing my boyfriend at Coleman 1?

Any tips on filling this thing out? Any tips would be apprecieated I am super new to this whole process and am so nervous at the thought of rejection!!!!!
Help!!!!
