Birth certificate for a visit ?

Hello, my man sent me the application for visitation privilege and they are requesting my birth certificate ? ! Does that make sense ? Also we have a son but he doesn't have his last name and he's not on his birth certificate because he was away when our son was born, does that mean I won't be able to take him ? Some ladies answered an older post and said it was no problem but is it different at Wilcox ? Thanks !

i had to have my birth cert. notarized along with my licens wewn sent form back in....and ur son has to get approved as well even though hes a baby
