Default Question- Where do I mail the visitation form?

Who do I mail the visitation form to and to what address? Never did this before, do not know what to expect.

Assuming you'll be visiting a CA prison, you send the form to the prison, marked ATTN:Visiting.

Example:

ATTN:Visiting
Shawshank Prison
POBox 1234
Anytown, CA 99999

you can also send it directly to the inmate and they will take it out before he gets it.

Default So many questions

I am 48 years old, and I got in trouble when I was 18, had to do I think 4 weekends in Jail, do I leet them know?

YES... let them know EVERYTHING!!! If you leave anything out and they find it, then you will be DENIED!!!

If you do not know exact dates, that is fine. Just put the county and the approx. year it happened (I could not remember days or years my stuff happened). This lets them know you are not trying to hide anything.

Good Luck to you!!

Oh, also, please make sure your loved one SIGNED the form before it got sent to you!! If not, then they will need to send you a new one that they sign FIRST, then you fill out and sign. The form WILL get rejected (not denied, per say) if the inmate has NOT signed it.

Again, this is a CALIFORNIA prison.

My forms explicitly stated NOT to send it to the inmate as they will NOT be accepted. Mine must go to the counselor.

Check out his prison forum....at mine there is a specific address to send it to. You can't send it to him so check before mailing it out I say

In California you can enclose it in a letter to him and the mail room will take it out and get it to visiting (the inmate never see's it). Like Patswife said, be sure he signs it! The signature line on the form isn't real obvious...but it's there. (Found out the hard way, lol).
