Default Hep C trials lead to high CURE rate

http://www.reuters.com/article/2012/...8MA0VL20121110

This is just amazing! I do hope the numbers stay as high as they're showing, especially for first line treatment (though the low/mid 90's for use as second and later line treatments is nothing to sneeze at, that's for sure). Would be nice to actually have people cured of Hep C, especially before there's significant liver damage. 3 months of drug treatment to cure - I'm keeping all of my appendages crossed that it stays this high or goes higher, it gets approved to move from trials to general treatment, and the side effects stay low (right now, only 1% quit treatment due to side effects - that's pretty decent).

Good news? Please! We need it. Cure type good news - even better!

I am delighted that there may be hope available since the rates of Hep C.in inmate populations is staggering and it is an awful way to die.
My loved ones father died of Hep. C. and he contracted it as well.
I also have a friend whose loved one is affected.
Thank you for this news!

The standard of care is changing! There are some dedicated doctors and scientists dedicated to improving SCR (sustained clinical response) and cure rate.

I have Genotype 1a which is the most resistent to therapy. New medications are expected to be FDA approved in December and available by spring. Once available, it will be 4 different pills and no Interferon. This will not be for everyone.

Diagnostics is also changing. The gold standard biopsy will soon be replaced with blood work and MRI. Biopsy results are not always accurate and have associated risks that a simple blood test does not have.

I am lucky enough to be treated at the MAYO Clinic. My doctor is the Key Opinion leader for this disease. This week, I am having the tests done to determine which treatment option is best for me. In the absence of cirrhosis, doctors are seriously in favor of doing nothing until the new 4 pill program is available.

Just last year the BOP defined Standards of Care for Inmates with Hep C. It is impossible to know if they will update their plan, but I would not be surprised if they also opt to wait for inmates with Genotype 1a who do not have cirrosis. The new treatment is NOT cheaper- just less risk and much better outcomes.

The new, highly promising, very much anticipated drug has been placed on FDA hold due to unacceptable adverse events. There is a 24 month hold to see if this can be worked out. The entire medical community was waiting with baited breath for this new treatment that has more work to be done.

Well, crap! How serious are the adverse effects? How serious do they need to be when the alternative is death?

Well, the sad truth is that while killing the virus, it was also destroying liver cells, making transplant the only option! Soooo they had no choice- 24 month delay started August 6th

I am very lucky! Throughout this journey, I have been able to always say that I have had the very best of medical attention. I have wonderful doctors who will do all that they can and I try very hard to be a good patient. I feel sorry for those who have improper medical care and a poor plan. I am lucky and hopeful

That's an impressive paper! Now, how long until you might be able to get the treatment? I know, I know, testing, testing, testing . . . .

Once this part of the study is over, and they are anticipating it to be this week, they will then look at all of the data, record the data and then make a decision to go to the FDA for approval or continue to evaluate. If the safety profile is good and the efficacy is good, they can ask for fast track approval, and probably get it putting it ahead of the one on hold. This would be huge for the patients and huge for the manufacturer.

They would also have to show that they have the ability to supply enough of the compound for the patients who would need it and I am not sure if they have done that benchmarking yet. The FDA is very strict on that and a company can get huge fines for marketing a drug and not having adequate supply for all of the patients.

Typically, Phase III would be the next step, but many blockbuster drugs do not have to do Phase III if they get fast track approval. This looks like it might get that. The only problem I can see is that a study of 70 subjects is plenty for Phase II, but deficient for Phase III. That said, if they can get approval for fast track launch, they have 18 months to document the results that would have been found in Phase III.

The fact that they have gone public with these results leads me to believe that they are doing the benchmarking, and hoping to gain fast track approval and if they do, I would guess availability by December.

December is not going to happen. The latest target date has been moved to next April. They have apparently been working on the most serious of the side effects, like liver failure and feel that they are on the right track. Lets hope so because in the meantime, the other 2 options have shown poor response

It would be great if something could put me back in remission! That would be awesome but it seems like I just have to wait and wait. I am lucky though because I do have wonderful doctors for this. Not everyone can say that, so I feel very lucky.

That one worked!

Interesting. When do they plan more trials, or are the stated positive results likely to allow early FDA approval?

They are going to try to fast track it through the FDA. That is always a gamble but worth trying.
