Default Minor Visitation Application

Hello,
I need to fill out an application for a 9 year old, 7 year old, and 9 month old. I only see a minor application for anyone over 13 and an adult application on the DOC website. I did see something about getting a form notarized for minors, but I do not see where exactly I need to apply for them! Any help would be appreciated

Here's what the Washington DOC says about minor visitors.

Minors, or anyone under the age of 18 must be on the inmates approved visitors list, be accompanied during their entire visit by a designated escort or parent/legal guardian, a copy of their original birth certificate must be attached to the online visiting application, and DOC 20-441 parent/guardian approval for visiting form.

Thank you. However, I am still unsure which online visiting application I am supposed to attach this to. There are two: first one is for adults and second one is for minors who are 13+ who can fill it out for themselves. All 3 children are under 13.

I would call the visitors application place, or, if you can't find that, pretty much any of the prisons. They'll be able to tell you what you need.

It's still not a visitor's application. I find WADOC to be one of the more difficult DOC's to find information on - their website does not have all the things that a person might need on it.

I hope the original poster finally found out how to do this and will update us.
