Do a search (Google / Yahoo) for a printable Durable Power of Attorney. Print it out, send it to him to be signed and notarized then sent back to you. In order to do certain things (banking) I had to take mine to the courthouse to be "Filed." That cost me $17.00, but its stamped by County Clerk and on file...they mailed me an "official" copy. Depending on what you need it for, you may or may not need to have it Filed at the Clerk's Office.
