I haven't but a friend of mine did. It's relatively quick and painless. He can get the forms at the facility and have them signed and notorized. Not sure if there is a cost...sometimes, sometimes not. He can also talk to the folks in the law library or his counselor. Make sure he sends you a copy (the original). He should also keep a copy. You make plenty of copies because everyone you talk to will want a copy.

Yes it is very easy & simple. Depends on facility though on how quick. He needs to get into the law library to get the form, fill it out and if he's lucky a notary is on hand where he is. If not, then he will have to kite for a notary and wait for that. Once notarized then he sends you it. As advised, make copies for your records. I personally keep 1 on me (purse) and 1 (original) locked up safe. If I need it, I make a copy of the original then put original back in it's safe place.

I got my forms at staples. There were three sets in the pack. I filled it out and sent it to him. He signed and had it notarized ( easy ) then sent it back. That's all it took. It's a good idea to put it in place. I was able to do what was needed without any issue. Even with the courts.
Good luck!
