Question Background Check/Visitation Approval Form?

Hi! I have been looking all over thinking that someone once posted the forms we have to fill out to get approved for visitation/background check. Am I imagining that it was posted? If we dont have it, is there somewhere I can get it from, other than my son (who doesnt have any stamps right now!) I looked on the azcorrections site but couldnt find it there either!

Default Az DOC Inmate Visitation Application

You can find the application at: azcorrections.gov on the extreme left side of the main poge under "Inmate Visitation Application" (3rd one up from the bottom). You can submit your allpication on-line which I would think would speed up the approval process. Hope this helps.

Has anybody else seen this? If you click on that link, apparently you can submit your application electronically now provided he/she are assigned to a state institution only. Contract beds here in AZ and OK you still have to fill it out and send it in.

I wonder if that will help speed things up with the approval process...or, maybe it will slow things down even more...hard telling!

In any event, there's the link...if you file your application electronically, be sure to tell us how that works out! Good luck!

OOPS...I just saw the lady who posted above me mentioned about the online application too...I was just excited!

Yes, it would speed it up providing the inmate has already added you to his/her 20 list. They don't even start the application process until the visitors are on the list.

Thank you all so much! My best friend, and second mother to my son, filled out the application my son sent her about 8 months ago. She called once,some months ago, to see if they were getting close to approving. They said because she had a very common name that it would take longer to be approved. She called back recently and they said they couldnt find her app!!!!

Doing it online will t least will save some time not waiting for my son sending it to her!

just some advice: i would def. call the unit after a week or so to verify that they received your app online. since it goes online now they can easily say they never received it, and that will delay your process even longer. but i hope it does make the process faster, because the 2.5 or 3 months its been taking lately are ridiculous!

Yes the visitation staff told me that there are still a lot of glitches in their new online application system..they suggest to have the form mailed in because sometimes they never get the online ones..because of the glitches..they said it was because the system is still too new..

So I wouldn't take any chances...send one in the mail regardless..you can still try submitting an online one...but to be safe send one through the regular mail as well..

I was just told this, this week..
