CDCR Form # 7385 – Release of Medical Info
Must be in Unit Health Record

Form must have current info including:
1. Name of Designated person approved to get info
2. Current address and phone number

3. Email address if available

When Designated person calls access line they must:
1. Give inmates Name, CDC # and DOB
2. Their phone # and address to confirm who they
are and for contact info.

Calipatria Health Care Staff will return call within 5 days

FOR SERIOUS HEALTH ISSUES ONLY

Exclamation Facility Changes & Appt. Changes

FROM CDCR VIA LIFE SUPPORT ALLIANCE:

For those with loved ones in California State Prisons the facilities are changing custody levels and the new conversions are taking place as follows:

This month SATF- F yard conversion. F yard will become Level II EOP Extended Outpatient , Facility D in Donavan will be a Level III SNY yard.

In October High Desert reception center yard A will be Level III GP, Valley State for Women will be converted to a men's prison and be renamed they are probably going to make it Level II SNY, most of the women will be sent next door and the conversion will be gradual with men and women both being housed there for a brief time. Men to be housed on A yard first and be completed in two months. CMF-Vacaville now a level III to become a level II.

In January Old Folsom now level III will become Level II GP.
In February CTF-North now Level III Sny to change to Level II.
Ironwood changing to Level IV SNY Facility A to Level 3 SNY in March.

In April Centinella will move Level IV GP Facility C to level II GP and DVI Tracy will convert 4 yards Level III GP to Level II
In May Alpine section of San Quentin will move from reception center Level II GP . Also reported that some institutions including CMC San Luis Obispo and CIM Chino is designated for prisoners with chronic but not acute medical issues resulting in prisoner transfers from several facilities. CRC Norco will be closed by fiscal year 2015/2016.

Starting next month all institutions go to visiting appointments from the CDCR website and phase out direct appoints made at the facility in compliance with the new SOMS statewide system.

Documents Allowed in Visiting

Documents up to 10 pages, no larger than 8- 1/2" by 11" (standard size typing paper); usually such documents will be either papers for the prisoner’s signature (for example, tax forms), information to share with the prisoner (for example, pages showing classes available through a correspondence course), or family papers (for example, a child’s report card, certificate of achievement, or drawing), but they can be anything that can be sent to the prisoner through the mail. Documents will be viewed and read by staff during processing and must be taken out by the visitor at the end of the visit.

Default New VPASS Statewide Visitor Appt. System

From Life Support Alliance July Newsletter - LSA is run by two former members of the Statewide IFC.

Visitor Processing Appointment Scheduling System (VPASS), is "the latest application being developed by CDCR. This online application will allow the department to offer tools to the public to enhance their visiting experience," according to CDCR’s informational handout. This new, statewide online appointment system will replace any and all online appointment processes now in place at many individual institutions (and often working well).

Although many institutions have implemented visiting appointment systems and several allow on-line appointment access, not every prison in the state has embraced visiting by appointment and many, especially those several prisons in the most remote, out-of-the-way locations, who do not experience the same numbers of visitors as other locations in only slightly-off-the-beaten-path spots, do not require appointments for mainline visiting at all. For those prisons with an existing on-line presence, the simple systems seem to work well.
VPASS, once operational, will be accessed not through email, but via the CDCR website. The department plans to include an "Institutional Banner Message" on the site, to provide up-to-date information on visiting status at the prison. Once on the VPASS page on the CDCR website visitor-hopefuls will be directed through a series of 7 pages or drop-down menus to (hopefully)make an appointment.


Originally slated to roll out at the end of May to coincide with the use of the SOMS computer program for visiting, VPASS was postponed and is now said to be ready to go live statewide in September. Stay tuned.

Yes you are but you have to take it off to go through the metal detector I believe
